I’ve Tested Positive for COVID-19 What Should I Do?

 

If you tested positive on a home test: Go to https://warrencountyny.gov/COVIDHomeTestReport and complete the online form. Your information will be sent to the New York State CommCare system for follow up.

If you test positive on a COVID test administered by a healthcare provider or school: Your information will be sent by the testing facility to the New York State CommCare System for follow up.

What is CommCare? CommCare is a program used by New York State to gather information about a positive COVID person and their close contacts. It uses email, text messages and phone calls to provide isolation and quarantine information and help with contact tracing.

Age Groups for Special Consideration: If you are 64 years old or older or you are less than 19 years of age you should expect a follow-up phone call from the CommCare system in addition to any text messages you may receive.

What type of message should I expect from CommCare? If you receive any text messages from CommCare it will include the follow

“NYS Department of Health is contacting __________. You tested positive for COVID-19. You should immediately isolate yourself, stay home for at least 5 days, and until you are fever free for 24 hours and symptoms are improving, followed by 5 days of wearing a mask when around others. Protect those you live with by staying 6 feet away when you are in the same room, wearing a mask, wash hands frequently, coughing into an elbow or tissue. Everything is kept confidential, and we will never ask for SSN or financial information. Notify your household and close contacts to immediately quarantine. Seek additional guidance from https://Ny.gov/isolation-quarantine-guidance , https://Ny.gov/isolation-quarantine-guidance-FAQ , https://www.cdc.gov/coronavirus/2019-ncov/your-health/quarantine-isolation.html . Replies are not monitored. Text STOP to opt out.”

How will I receive my isolation or quarantine orders for work or school?

If you have been told that you have tested positive for COVID-19 or have been told you are considered a close contact with a COVID-19 positive person and you have not been provided with information for your isolation/quarantine orders you should use one of the follow websites to complete the online isolation and quarantine documents
